kubeadm: add configuration option to not taint master

**What this PR does / why we need it**:

Although tainting the master is normally a good and proper thing to do in some situations (docker for mac in our case, but I suppose minikube and such as well) having a single host configuration is desirable.

In linuxkit we have a [workaround](443e47c408/projects/kubernetes/kubernetes/kubeadm-init.sh (L19...L22)) to remove the taint after initialisation. With the change here we could simply populate `/etc/kubeadm/kubeadm.yaml` with `noTaintMaster: true` instead and have it never be tainted in the first place.

I have only added this to the config file and not to the CLI since AIUI the latter is somewhat deprecated.

The code also arranges to _remove_ an existing taint if it is unwanted. I'm unsure if this behaviour is correct or desirable, I think a reasonable argument could be made for leaving an existing taint in place too.

This PR enables [Auditing](https://kubernetes.io/docs/tasks/debug-application-cluster/audit/) behind a featureGate. A user can supply their own audit policy with configuration option as well as a place for the audit logs to live. If no policy is supplied a default policy will be provided. The default policy will log all Metadata level policy logs. It is the example provided in the documentation.

kubeadm: imagePullPolicy option in init config

**What this PR does / why we need it**:
This PR adds `imagePullPolicy` option to the `kubeadm init` configuration file.

The new `imagePullPolicy` option is forwarded to the generated kubelet static pods for etcd, kube-apiserver, kube-controller-manager and kube-scheduler. This option allows for precise image pull policy specification for master nodes and thus for more tight control over images. It is useful in CI environments and in environments, where the user has total control over master VM templates (thus, the master VM templates can be preloaded with the required Docker images for the control plane services).

Ed Bartosh 3ecc49daaa Add HTTPProxyCheck for API servers
It makes sense to check all API servers mentioned in the command
line and print warnings if they're going to be accessed through proxy.
This is similar to what's already done for 'kubeadm init'.

This lets us use omitempty on `EnforceNodeAllocatable`. We shouldn't treat
`nil` as different from `[]T{}`, because this can play havoc with

Remove port from HTTPProxyCheck

**What this PR does / why we need it**:

HTTPProxyCheck doesn't use port. It uses proxy related variables
(HTTP_PROXY, NO_PROXY, etc) that only operate with protocol
and host.

Removing port from the check should make it more clear for
user that port is not used as it will not be present in
the check output.

2nd try at using a vanity GCR name

The 2nd commit here is the changes relative to the reverted PR.  Please focus review attention on that.

This is the 2nd attempt.  The previous try (#57573) was reverted while we
figured out the regional mirrors (oops).
    
New plan: k8s.gcr.io is a read-only facade that auto-detects your source
region (us, eu, or asia for now) and pulls from the closest.  To publish
an image, push k8s-staging.gcr.io and it will be synced to the regionals
automatically (similar to today).  For now the staging is an alias to
gcr.io/google_containers (the legacy URL).
    
When we move off of google-owned projects (working on it), then we just
do a one-time sync, and change the google-internal config, and nobody
outside should notice.
    
We can, in parallel, change the auto-sync into a manual sync - send a PR
to "promote" something from staging, and a bot activates it.  Nice and
visible, easy to keep track of.

TL;DR:
  *  The new `staging-k8s.gcr.io` is where we push images.  It is literally an alias to `gcr.io/google_containers` (the existing repo) and is hosted in the US.
  * The contents of `staging-k8s.gcr.io` are automatically synced to `{asia,eu,us)-k8s.gcr.io`.
  * The new `k8s.gcr.io` will be a read-only alias to whichever regional repo is closest to you.
  * In the future, images will be promoted from `staging` to regional "prod" more explicitly and auditably.

Davanum Srinivas 18590378c4 Remove experimental keystone authenticator
experimental-keystone-url and experimental-keystone-ca-file were always
experimental. So we don't need a deprecation period.
KeystoneAuthenticator was on the server side and needed userid/password
to be passed in and used that to authenticate with Keystone. We now
have authentication and authorization web hooks that can be used. There
is a external repo with a webook for keystone which works fine along
with the kubectl auth provider that was added in:
a0cebcb559

So we don't need this older style / hard coded / experimental code
anymore.

Do not recycle volumes that are used by pods

**What this PR does / why we need it**:
Recycler should wait until all pods that use a volume are finished.

Consider this scenario:

1. User creates a PVC that's bound to a NFS PV.
2. User creates a pod that uses the PVC
3. User deletes the PVC.

Now the PV gets `Released` (the PVC does not exists) and recycled, however the PV is still mounted to a running pod. PVC protection won't help us, because it puts finalizers on PVC that is under user's control and user can remove it.

This PR checks that there is no pod that uses a PV before it recycles it.

IPv6: Ensure calculated node CIDR size for pod subnets is valid

With IPv4, the node CIDR prefix is set to /24, which gives 256 pods per node
and 256 nodes, when assuming a /16 is used for the pod subnet.

For IPv6, the node CIDR prefix, is hard coded to /64. This does not work,
because the pod subnet prefix must be /66 or higher and must be a larger subnet
(lower value) than the node CIDR prefix.

In addition, the bit mask used to track the subnets (implying the number of
nodes), can only handle 32K entries, so the difference between pod subnet
prefix and node CIDR prefix cannot be more than 16 (bits).

To address this, the following algorithm is proposed to provide as many pods
per node as possible, and not exceed the number of nodes.

If the pod subnet prefix is from /66 to /104, we'll set the node CIDR prefix
to 16 more, so that the bit map is not exceeded. The rest of the bits will be
for pods per node.

If the subnet prefix is from /105 to /112, we'll split the available bits
between what is used for the nodes and what is used for the pods per node.
This will give a node CIDR prefix from /116 to /120.

If the subnet prefix is from /113 to /119, we'll do like IPv4 and ensure that
there are 256 pods per node, and the remaining bits will be for the nodes,
giving a node CIDR prefix of /120 always. This supports a limited number of
nodes, in some cases.

If the subnet prefix is /120 to /128, we don't have enough bits and will set
the node CIDR prefix to be the same as the pod subnet prefix. This will cause
a falure later, when it tests that the pod subnet prefix is larger than the
node CIDR prefi.

Paul Michali fcd0ab1f6e IPv6: Ensure calculated node CIDR size for pod subnets is valid
With IPv4, the node CIDR prefix is set to /24, which gives 256 pods per node
and 256 nodes, when assuming a /16 is used for the pod subnet.

For IPv6, the node CIDR prefix, is hard coded to /64. This does not work,
because currently the pod subnet prefix must be /66 or higher and must be a
larger subnet (lower value) than the node CIDR prefix.

In addition, the bit mask used to track the subnets (implying the number of
nodes), can only handle 64K entries, so the difference between pod subnet
prefix and node CIDR prefix cannot be more than 16 (bits). The node CIDR
value needs to support this restriction.

To address this, the following algorithm is proposed...

For pod subnet prefixes of /113 or smaller, the remaining bits will be used
for the node CIDR, in multiples of 8, and 9-16 bits will be reserved for the
nodes, so that there are 512-64K nodes and 256, 512, 768, ... pods/node.

For example, with a pod network of /111, there will be 17 bits available. This
would give 8 bits for pods per node and 9 bits for nodes. The node CIDR would
be /120. For a pod network of /104, there will be 24 bits available. There will
be 8 bits for nodes, and 16 bits for pods/node, using a /112 node CIDR.

If the pod subnet prefix is /112, then the node CIDR will be set to /120, and
256 nodes and 256 pods/node will be available.

If the subnet prefix is /113 to /128, we don't have enough bits and will set
the node CIDR prefix to be the same as the pod subnet prefix. This will cause
a falure later, when it tests that the pod subnet prefix is larger than the
node CIDR prefix.

Cap how long the kubelet waits when it has no client cert
If we go a certain amount of time without being able to create a client
cert and we have no current client cert from the store, exit. This
prevents a corrupted local copy of the cert from leaving the Kubelet in a
zombie state forever. Exiting allows a config loop outside the Kubelet
to clean up the file or the bootstrap client cert to get another client
cert.

Fix adding FileContentCheck

Current code adds FileContentCheck only for the first API
Server mentioned in the command line. The test is never added
as net.ParseIP always fails because address:port is passed
to it instead of just an address.

Fixed both issues by introducing a loop over all API Servers
and splitting address:port before passing address to the
net.ParseIP API.

Removal of KubeletConfigFile feature gate: Step 1

This feature gate was redundant with the `--config` flag, which already
enables/disables loading Kubelet config from a file.

Since the gate guarded an alpha feature, removing it is not a violation
of our API guidelines.

Some stuff in `kubernetes/test-infra` currently sets the gate,
so removing will be a 3 step process:
1. This PR, which makes the gate a no-op.
2. Stop setting the gate in `kubernetes/test-infra`.
3. Completely remove the gate (this PR will get the release note).

This is to allow the use case where cluster administrators wish
to limit the pids consumed per pod (example when running a CI system).

By default, we do not set any maximum limit, If an administrator wants

`--feature-gates=` parameter to kubelet and specify the limit using the
`--pod-max-pids` parameter.

The limit set is the total count of all processes running in all
containers in the pod.

serviceaccount: check token is issued by correct iss before verifying

Right now if a JWT for an unknown issuer, for any subject hits the
serviceaccount token authenticator, we return a errors as if the token
was meant for us but we couldn't find a key to verify it. We should
instead return nil, false, nil.

This change helps us support multiple service account token
authenticators with different issuers.

uniquify resource lock identities

Resource lock identities on the same host were not unique.  This keeps the hostname (that's valuable), but also adds a UUID so that you can start a process twice on the same host and not end up with two processes trying to lead.

Ed Bartosh d503b807d5 Fix adding FileContentCheck
Current code adds FileContentCheck only for the first API
Server mentioned in the command line. The test is never added
as net.ParseIP always fails because <address>:<port> is passed
to it instead of <address>.

Fixed both issues by introducing a loop over all API Servers
and splitting <address>:<port> before passing <address> to the
net.ParseIP API.

Michael Taufen 6443b6f543 Removal of KubeletConfigFile feature gate: Step 1
This feature gate was redundant with the `--config` flag, which already
enables/disables loading Kubelet config from a file.

Since the gate guarded an alpha feature, removing it is not a violation
of our API guidelines.

Some stuff in `kubernetes/test-infra` currently sets the gate,
so removing will be a 3 step process:

dockershim: remove the use of kubelet's internal API

We let dockershim implement the kubelet's internal (CRI) API as an
intermediary step before transitioning fully to communicate using gRPC.
Now that kubelet has been communicating to the runtime over gRPC for
multiple releases, we can safely retire the extra interface in
dockershim.

This PR also moves the legacy functions to a separate file and clean up
the interfaces.

Default Go HTTP transport does not allow to use CIDR notations in
NO_PROXY variables, thus for certain HTTP calls that is done inside
kubeadm user needs to put explicitly multiple IP addresses. For most of
calls done via API machinery it is get solved by setting different Proxy
resolver. This patch allows to use CIDR notations in NO_PROXY variables
for currently all other HTTP calls that is made inside kubeadm.

Davanum Srinivas 8e49774f90 Enable privileged containers for apiserver and controller
In OpenStack environment, when there is no metadata service, we
look at the config drive to figure out the metadata. Since we need
to run commands like blkid, we need to ensure that api server and
kube controller are running in the privileged mode.

So add a new field in MasterConfiguration for specifying that the
api server and controller manager (s) need extra privileges. Added
a TODO to remove this code when we fully yank out cloud provider
specific calls from these processes.

Support for custom tls cipher suites in api server and kubelet

**What this PR does / why we need it**:
This pull request aims to solve the problem of users not able to set custom cipher suites in the api server.
Several users have requested this given that some default ciphers are vulnerable.
There is a discussion in #41038 of how to implement this. The options are:
- Setting a fixed list of ciphers, but users will have different requirements so a fixed list would be problematic.
- Letting the user set them by parameter, this requires adding a new parameter that could be pretty long with the list of all the ciphers.

I implemented the second option, if the ciphers are not passed by parameter, the Go default ones will be used (same behavior as now).

**Which issue this PR fixes** 
fixes #41038 

**Special notes for your reviewer**:
The ciphers in Go tls config are constants and the ones passed by parameters are a comma-separated list. I needed to create the `type CipherSuitesFlag` to support that conversion/mapping, because i couldn't find any way to do this type of reflection in Go.
If you think there is another way to implement this, let me know.

If you want to test it out, this is a ciphers combination i tested without the weak ones:
