This PR add comments for the background why plugin gets loopback
device and removes loopback device even if operation_generator has
same functionality.
WaitForAttach failed consistently with this error:

no such file or directory
We should create dir first to avoid the error.

After a volume is unmounted from pod and worker node,
cluster admin or external-provisioner might delete
the disk from storage, therefore block device on
the node should be cleaned up beforehand.
The photon volume plugin already has same functionality.

A minimalistic multipath.conf got written, but it was useless, as
it is unclear if multipathd is running and there was also no
config reload triggered.
This patch drops this snippet. In general it's probably a better idea
to leave the multipath.conf to the component managing the host.

If it does belong to the device then we make sure we mount the mpio device instead of
the raw device.
Heuristics
Login into /dev/disk/by-path/iqn-example.com.2999 -> /dev/sde
Check if sde existsin in /sys/block/[dm-X]/slaves/xx
If it does mount /dev/[dm-x] which will look like /dev/mapper/mpiodevicename in mount
examples/iscsi has more details
