Retry certificate approval on conflict errors

We already check preconditions. We were seeing a non-trivial number of conflicts when using the command from automation.

Clayton Coleman c7b37767ba
Make certificate approve/deny no-op if CSR is already approved 
Approval for CSRs is only needed once. If the CSR is already approved or
denied we can skip updating it. Add a `--force` flag that allows the
existing behavior for when a user has a specific need to update the CSR.

This is backwards compatible with the intended use of the conditions,
although it's possible some users are depending on the status being
updated. It makes bulk approval as an admin much faster for scripting.

All commands must declare Unstructured or Internal 
Callers must take a dependency on one or the other set of conversions
and default client behavior. Future changes may add a Versioned() type,
but this is an accurate reflection of current code state.

add Local and Unstructured builder attributes 
Moves DisabledClientMapperForMapping wrapper to new Local attribute.
Removes Factory#NewUnstructuredBuilder in favor of new Unstructured
builder attribute.

* Rename variables and functions to match Go convention.
   For example, UsageError --> UsageErrorf.
 * Remove redundant or unreachable code.
 * Simplify some utility functions (no functionality changes).
 * Fix hanging 'if { return } else { return }' constructs.
 * Fix several incorrect printf verbs.
